By registering my child for the White Hill camp in 2020, I as a parent/guardian of the applicant hereby give permission for my child to participate in Luis Quezada’s USA Soccer Camp and agree to comply with all of its rules and regulations. We and/or I recognize the possibility of physical injury associated with soccer and other sport activities, and herby release, discharge, and otherwise indemnify Luis Quezada’s USA Soccer Camp, San Anselmo Dept. of Parks and Recreation, Ross Valley School District, West Marin Youth Soccer League, Camp Coaches, their sponsors, affiliated organizations, and the employees and associated personnel of these organizations, against any claim by or on behalf of the participant named as a result of that player’s participation in the USA Soccer Camp and or being transported to or from the same, which transportation I hereby authorize.

Luis Quezada’s USA Soccer Camp may use photos and videos taken at their events for promotional, non-commercial purposes. These may be published on the camp website, in brochures, newspapers, and other youth soccer-related forums at USA Soccer Camp’s discretion. By registering a player with USA Soccer Camp programs, I agree to the terms of this Media (Photo/Video) Policy. Photos and videos of players will be removed upon request by contacting the Luis Quezada’s USA Soccer Camp.
